What is Ferritin and How to Track It?

Recommended Tracking Cadence
Every 3 to 6 months (or 8 weeks after iron supplementation)

1. What is Ferritin (Iron Storage)?

Ferritin is the primary intracellular protein that stores iron and releases it in a controlled fashion. It is the most sensitive early marker of total-body iron reserves—and also acts as an acute-phase reactant when systemic inflammation is present.

2. Why Track Ferritin?

  • Low ferritin (<30–40 ng/mL) causes unexplained fatigue, hair shedding, restless legs, and reduced VO2 max even when Hemoglobin is still "normal".
  • Elevated ferritin (>200–300 ng/mL) can signal oxidative iron overload, fatty liver stress, or hidden inflammation.
  • Standard lab reference minimums (often 12–15 ng/mL) are far below the physiological threshold needed for optimal thyroid conversion and mitochondrial energy.

3. How to Track Ferritin Accurately

  • Always interpret Ferritin alongside hs-CRP (to rule out inflammation-driven spikes) and a Complete Blood Count (Hemoglobin, MCV, RDW).
  • Test in the morning in a fasted state, avoiding strenuous endurance exercise 24 hours prior.
  • If supplementing iron, re-test 8 to 12 weeks later after pausing iron supplements for 48 hours.

4. How to Optimize Ferritin

  • If low: pair heme iron (grass-fed beef, sardines, liver) or iron bisglycinate with Vitamin C, and separate from coffee/tea by 2 hours.
  • If high with normal transferrin saturation: address metabolic inflammation, alcohol intake, and hepatic insulin resistance.
  • If high with high transferrin saturation: consult your physician regarding therapeutic blood donation or HFE screening.

5. Frequently Asked Questions

Can I have iron deficiency with normal hemoglobin?

Yes. Non-anemic iron deficiency occurs when Ferritin drops below 35–45 ng/mL before red blood cell hemoglobin declines.

Why does inflammation raise Ferritin?

During infection or metabolic inflammation, the body sequesters iron inside cells, pushing serum Ferritin higher even if usable iron is low.
